Cynthia Beaulieu, PhD, ABPP-CN is a board-certified clinical neuropsychologist and Director of Brain Injury Development at Brooks Rehabilitation Hospital in Jacksonville, Florida. Dr. Beaulieu is a member of the clinical medical faculty at the University of North Florida, conducts collaborative research with colleagues from the University of Florida on topics of trauma and rehabilitation, and has been the on-site study coordinator on the large scale Traumatic Brain Injury Practice-Based Evidence study funded through NIH and NIDRR. Dr. Beaulieu earned a BS in psychology from the University of Michigan, and a MA and PhD in clinical psychology from the Ohio State University.

Within the last 24 months, have you rendered, or in the next two years, will you render clinical services or support services (whether diagnostic, therapeutic, educational, case management, or similar) to persons with TBI or their family members and caregivers, as an independent professional or employee of a for-profit or not-for-profit organization?: I am a board-certified clinical neuropsychologist who performs evaluations and treatments to survivors of TBI on an Inpatient brain injury unit within an acute inpatient rehabilitation hospital (Brooks Rehabilitation Hospital). These clinical services are provided as fee-for-service. My position is salaried, thus I do not personally benefit from the revenues generated from my clinical services. I perform clinical service <2% of the time during my typical workweek.

Within the past 24 months, I or one of my “immediate family members” received personal compensation for the following: Brooks Rehabilitation Hospital reimburses my travel costs and expenses for annual conferences that I attend for continuing education, research presentations on topics related to TBI, and marketing and development for the Brooks Rehabilitation BI Program; My position in the past years and moving forward is designated as Director of Brain Injury. I provide the overall direction of program development for the Brooks Rehabilitation BI Program.
