Within the last 24 months, have you rendered, or in the next two years, will you render clinical services or support services (whether diagnostic, therapeutic, educational, case management, or similar) to persons with TBI or their family members and caregivers, as an independent professional or employee of a for-profit or not-for-profit organization?: I am the Executive Director of On With Life Inc., a non-profit organization specifically dedicated to providing rehabilitation services to person with brain injury. In my capacity, I provide overall leadership for the programs 100% of my time. I no longer provide direct services to persons served or family members. My compensation will not increase as a result of any specific guidelines developed.

Within the past 24 months, I or one of my “immediate family members” received personal compensation for the following: CARF surveyor since 2000; travel is provided; Executive Director, On With Life Inc.; Secretary of the Corporation, On With Life Ltd.; Director, On With Life Supporting Housing Corp.; Director, On With Life Foundation.

Within the past 24 months and during the three-year course of the project under consideration, I or one of my immediate family members received or will receive financial or material research support or compensation from the following: My husband, Dr. Dennis Dixon, taught classes for Simpson College and Des Moines Area Community College through 2013.
